Application for endorsement of removal service plan

Method 1: Submit the application and the documents required through Regulated Electrical Equipment Information Management System (REEIMS)

Step 1: Register a REEIMS Account
After entering the system, click "Do not have a user account? Register Now". You are required to fill in the following information and upload the following documents:

1. particulars of the company
2. a copy of the valid Business Registration Certificate
3. a duly completed "Agreement to Use Regulated Electrical Equipment Information Management System" (click here to download)

Step 2: Apply for Endorsement of Removal Service Plan
After completing Step 1, we will send you an invitation email. Please click the link in the email and follow the instructions given by REEIMS to fill in the following online forms:

• Seller's Application for Endorsement of Removal Service Plan (REE-5) and Supplementary sheet for seller's distribution avenues (TREE-3)

and then upload the following documents onto REEIMS:

1. a copy of the valid Business Registration Certificate
2. a copy of the owner's valid Hong Kong Identity Card (HKID Card) (if the applicant is a sole-proprietorship company)
3. a copy of the valid HKID Card or travel document for individual partners (if the applicant is a partnership company)
4. Removal Service - Undertaking of a Collector (REE-5a)
5. a copy of the valid Business Registration Certificate for each collector specified in the removal service plan
6. a copy of the valid HKID Card of the owner or all partners for each specified collector (if the collector is a sole-proprietorship or a partnership company)
7. Removal Service - Undertaking of a Recycler (REE-5b)
8. a copy of the specified recycler's valid E-waste Disposal Licence (part A and part B) issued under the Waste Disposal Ordinance (Cap. 354 of the Laws of Hong Kong)
9. Removal Service Terms and Conditions prepared by the seller (may make reference to Removal Service Terms and Conditions Template (TREE-9))
10. a copy of the valid Business Registration Certificate for each branch of the seller (i.e. Branch Registration Certificate)

Method 2: Duly complete the following specified forms and submit them together with the following supporting documents

1. Seller's Application for Endorsement of Removal Service Plan (REE-5) and Supplementary Sheet for Seller's Distribution Avenues (TREE-3);
2. a copy of the valid Business Registration Certificate
3. a copy of the owner's valid HKID Card (if the applicant is a sole-proprietorship company)
4. a copy of the valid HKID Card or travel document for individual partners (if the applicant is a partnership company)
5. Removal Service - Undertaking of a Collector (REE-5a)
6. a copy of the valid Business Registration Certificate for each collector specified in the removal service plan
7. a copy of the valid HKID Card of the owner or all partners for each specified collector (if the collector is a sole-proprietorship or a partnership company)
8. Removal Service - Undertaking of a Recycler (REE-5b)
9. a copy of the specified recycler's valid E-waste Disposal Licence (part A and part B) issued under the Waste Disposal Ordinance (Cap. 354 of the Laws of Hong Kong)
10. Removal Service Terms and Conditions prepared by the seller (may make reference to Removal Service Terms and Conditions Template (TREE-9))
11. a copy of the valid Business Registration Certificate for each branch of the seller (i.e. Branch Registration Certificate).
